Top Hospital: Errors in 1/2 of Surgeries

Researchers at Massachusetts General Hospital discovered that 124 of 277 operations they observed in 2013-2014 had at least one medication error or drug-related problem which harmed the patient. The errors included mistakes in labeling, wrong dosages, and medications that were not administered when they should have been. The findings were presented to the American Association of Anesthesiologists last weekend.
