Could an oil spill like the BP disaster happen again? It might unless the energy industry changes its ways, according to a new report released by the presidential commission investigating the affair. The seven-member panel blames a "failure of management" for what happened on the Deepwater Horizon drilling rig. BP and other companies were responsible for a series of "missteps and oversights," the panel says. Decisions were made by BP and others to save time and money which resulted in the spill, according to the commission's findings.
